Declaration of consent

I consent to my data being collected, processed and used by the University of Mannheim for the following purpose: investigation and resolvement of the security incident specified above. I have been informed that all data relating to me personally collected for the aforementioned purpose are collected, processed, used and transmitted pursuant to the Data Protection Act of the Land of Baden-Württemberg (LDSG), the German Federal Data Protection Act (BDSG), and the EU General Data Protection Regulation (GDPR). I have also been informed that my data are collected, processed and used on a voluntary basis and that I can withhold my consent and/or revoke it at any time with effect for the future. As a consequence, it will not be possible to issue the corresponding reports due to a lack of information. I will address the declaration to revoke my consent to:
